My great uncle served on that ship.  It's a Forrest Sherman class destroyer.  Revell made a kit of the USS Forrest Sherman which is what you want.  The released the same kit under a different name, but I don't remember that name.  Hope this helps.

There is a JAG collective 1/700 waterline Forrest Sherman. I think its owned by Admiralty Modelworks now.
