Try using Future as your setting solution. Works quite well.

99.9% of silvering is caused by the final construction phase.Area will have to be smooth and blemish free before a primer is applied.

All;

I will occasionally get a silvered decal and I am so careful to always use the same technique; gloss varnish over the paint (sometimes two coats), then using MicroSet, damp Q-Tip to roll and bleed off excess, then even a thin coat of MicroSol. 

I am wondering about putting a few drops of the gloss varnish into the warm water that I dip the decals in?  Perhaps some clear liquid glue like Elmers?  Anything to help give a liquid-only base for the decal to set into.  Darn those pesky air bubbles.  Thoughts?
