Thanks.  I have a brother who is trying to find that out from his logbook.  I'll keep in touch.

If you have the serial number of his UH-34D, I'd be happy to check my files for photos of it for you.

Unfortunately, the 1/72 Italeri kit is it.  There was an old Revell kit in 1/48th, but the molds were destroyed in an accident and it hasn't been seen since the early '80s.  Any surviving kits are collectors items and can go for quite a bit.

The Italeri 1/72 one can come out quite nicely though.  It just needs a little work.  You can see a fine example of how it can come out here.

Good luck.

My father is getting on in years, and I want to re-create for him the helicopter that he flew in Vietnam with HMM-161 squadron.  On 19 September 1965 he rescued the crew of two downed helo's and was awarded the Silver Star and Purple Heart.  I want to model the UH-34D that he used.

The Italeri UH-34 Sea Horse (1:72) seems to be the closest to it.  Is there a better one?  Maybe something in 1:48?
