The guy on the stretcher is merely having an identity crisis. He thinks he came out of a Tamiya factory......Clown

There is an additional problem with the casualty.  Look at how he is 'laying' on the stretcher.  He isn't - there is a gap from his heals to his head.  Sooooo, maybe he is actually dead and rigor mortis has set in?  (Actually, rigor wouldn't look that way...)

 

Now, Boyd, don't you know the Amazing  Kreskin and CrissAngel are the stretcher bearers and they're levitating the casualty!

i just figured he was arching in pain.

His back and legs are too straight for that....

I would have expected the wounded man to be a bit bloody and muddy.

The medics are also a bit "pristine" as well.

The streets look like they were meticulously cleaned.

The truck looks OK (except, again, for the lack of blood) but the rest of it just doesn't seem to have the necessary "dirty" look.
